How to Refer Under a Mental Health Treatment Plan (MHTP)
What should a referral include?
A mental health treatment referral should be a signed and dated referral and include:
patient's name
date of birth
address
symptoms or diagnosis
current medications
number of treatment services being referred for
confirmation that a Mental Health Treatment Plan or relevant plan has been prepared.
Number of sessions
Initial course: up to 6 individual services
Subsequent course: remaining services up to the annual limit
Annual limit: up to 10 individual Better Access services per calendar year.
A new referral is required for each course of treatment.

Psychological Assessment Referrals
We accept professional and self-referrals for psychological assessment. A Mental Health Treatment Plan is not required for psychological assessment referrals.
Referrers are welcome to include relevant clinical history, previous assessments, school reports or other information that may assist in clarifying the referral question.

Working with GPs
ELD Psychology values collaborative care with GPs and other treating professionals. Our psychologists:
Provide written correspondence regarding treatment as required
Communicates relevant treatment recommendations
Encourages collaboration with the GP where clinically appropriate
Assist patients with returning to their GP for further referral/review where required.
Under Better Access, psychologists are required to provide the referring practitioner with a written report following each course of treatment, including assessment, treatment provided and recommendations for future management.
